Step-by-Step Instructions

  1. Log in to your QRSpike account and go to your Dashboard.
  2. Click the Create QR Code button.
  3. Select Contact Form from the QR code type dropdown.
  4. Enter a name for your QR code. This name appears on your Dashboard for identification.
  5. Build your form using the Form Builder:
    • Use the Form Builder to add fields to your Contact Form. Available field types include text fields, email fields, phone fields, text areas, dropdowns, checkboxes, and radio buttons.
    • Drag fields to rearrange their order on the form.
    • Configure each field's label, placeholder text, and whether it is required.
    • Use the field settings panel to fine-tune each field's properties.
  6. Customize the QR code's Color, Background Color, Icon, and Border settings.
  7. Click Preview to see what the QR code looks like.
  8. Click Save to create your Contact Form and its associated QR code.

Downloading Form Submissions as CSV

As people fill out your Contact Form, their responses are collected in your QRSpike account. To access this data:

  1. Go to your Dashboard and locate the Contact Form QR code.
  2. Click on the QR code to view its details and submitted responses.
  3. Click the Download CSV button to export all form submissions as a comma-separated values file. You can then open the CSV in Excel, Google Sheets, or any spreadsheet application for analysis, mail merges, or CRM import.
