How to Create a Survey

Pair the survey URL with a QR code and distribute it on printed materials, signage, or digital channels. Respondents scan the code, fill out the survey, and their answers are stored in your QRSpike account.

Step-by-Step Instructions

  1. Log in to your QRSpike account and go to your Dashboard.
  2. Click the Create QR Code button.
  3. Select Contact Form / Survey from the QR code type dropdown.
  4. Enter a name for your QR code.
  5. Build your survey using the Form Builder:
    • Add fields for each survey question. Use text fields for open-ended questions, radio buttons for single-choice questions, checkboxes for multi-choice questions, and dropdowns for list selections.
    • Drag fields to arrange the question order.
    • Set labels to clearly describe each question.
    • Mark fields as required if a response is mandatory.
  6. Customize the QR code's Color, Background Color, Icon, and Border settings.
  7. Click Preview to see what the QR code looks like.
  8. Click Save to create your Survey and its associated QR code.

Downloading Survey Responses as CSV

As people complete your survey, their responses are collected in your QRSpike account. To access the data:

  1. Go to your Dashboard and locate the Survey QR code.
  2. Click on the QR code to view its details and submitted responses.
  3. Click the Download CSV button to export all responses as a comma-separated values file. You can open the CSV in Excel, Google Sheets, or any spreadsheet application to analyze trends, create charts, and draw conclusions from your data.
